This near chart is good for measuring the visual acuity within the near distance (40 cm). 

Functional details 

On the front side of this chart there are ETDRS sloan letters ("chart 1"). On the backside the optotypes are in a different order (for a second test - "chart 2"). The cord attached is made for keeping the needed distance of 40cm. The scales are in the metric system and snellen (feet), but also in decimal and logMAR indications of the visual acuity.
